Published service description
The Single Point of Access (SPA) is for people who require healthcare mainly in their own home in Bristol, North Somerset and South Gloucestershire. Our service sees people over the age of 18, or who are soon to be over 18 and are transitioning from children’s services. Visits can include wound care, catheter care, insulin administration, taking bloods, therapy and urgent community response referral.
Calls will be answered by our team of Health Navigators. For security, and in accordance with General Data Protection Regulation (GDPR) laws, we will ask for three bits of identifying information before proceeding with your request.
Depending on the urgency of the referral you may be passed to a Triage Clinician to review your care, or directed to your local Integrated Neighbourhood Team.
We are unable to provide visit times or confirm the names of nurses who will be visiting.
